Steps:

1. Log into startaz.io
2. Locate and click on the HR & Payroll module. Click on the HR Admin dropdown menu
3. Click on the Employment Termination link. This directs you to page displaying your employment termination details 
4. Click on the Action menu at the far right then select Take Exit Interview from the options presented
5. This redirects you to a form containing exit interview questions. Fill them accordingly then click on Save to complete
